TikTok-ified DiGAs? Nope, I am not Making This Up.

Just read this Handelsblatt piece: "A Touch of TikTok Doesn't Hurt." Dug deeper. Now, here we are.

💡 Starting 2026, DiGA manufacturers must report “application-related success measurement” (AbEM) metrics—and pricing will be tied to them.

The big 3:

  • ⌛ How often & how long patients use the app

  • 👍 Patient satisfaction (Did they like it?)

  • ⁉️ Self-reported health outcomes (Did it work?)

📢 Oh, and 𝟮𝟬% 𝗼𝗳 Pricing 𝘄𝗶𝗹𝗹 𝗯𝗲 𝗯𝗮𝘀𝗲𝗱 𝗼𝗻 𝘁𝗵𝗲𝘀𝗲 𝗺𝗲𝘁𝗿𝗶𝗰𝘀—but will it actually fix DTx adoption?

𝗛𝗲𝗿𝗲’𝘀 𝘁𝗵𝗲 𝗶𝘀𝘀𝘂𝗲: these metrics 𝘁𝗿𝗮𝗰𝗸 𝘀𝘂𝗿𝗳𝗮𝗰𝗲-𝗹𝗲𝘃𝗲𝗹 𝗲𝗻𝗴𝗮𝗴𝗲𝗺𝗲𝗻𝘁 but don’t address the real-world barriers stopping patients from using DTx.

📌 𝙁𝙚𝙚𝙡𝙨 𝙡𝙞𝙠𝙚 𝙚𝙭𝙩𝙧𝙖 𝙬𝙤𝙧𝙠—𝘸𝘩𝘰 𝘩𝘢𝘴 𝘵𝘪𝘮𝘦 𝘧𝘰𝘳 𝘢𝘯𝘰𝘵𝘩𝘦𝘳 𝘢𝘱𝘱?

📌 𝘿𝙤𝙘𝙩𝙤𝙧𝙨 𝙖𝙧𝙚𝙣’𝙩 𝙘𝙤𝙣𝙫𝙞𝙣𝙘𝙚𝙙——𝘵𝘩𝘦𝘺 𝘯𝘦𝘦𝘥 𝘤𝘭𝘦𝘢𝘳 𝘤𝘭𝘪𝘯𝘪𝘤𝘢𝘭 𝘣𝘦𝘯𝘦𝘧𝘪𝘵𝘴 & 𝘪𝘯𝘤𝘦𝘯𝘵𝘪𝘷𝘦𝘴.

📌 𝙈𝙤𝙨𝙩 𝘿𝙞𝙂𝘼𝙨 𝙖𝙧𝙚𝙣’𝙩 𝙗𝙪𝙞𝙡𝙩 𝙞𝙣𝙩𝙤 𝙧𝙚𝙖𝙡-𝙡𝙞𝙛𝙚 𝙧𝙤𝙪𝙩𝙞𝙣𝙚𝙨—𝘵𝘩𝘦𝘺 𝘫𝘶𝘴𝘵 𝘴𝘪𝘵 𝘵𝘩𝘦𝘳𝘦.

📌 𝙋𝙖𝙩𝙞𝙚𝙣𝙩𝙨 𝙨𝙩𝙞𝙡𝙡 𝙚𝙭𝙥𝙚𝙘𝙩 𝙖 𝙥𝙞𝙡𝙡, 𝙣𝙤𝙩 𝙖 𝙥𝙧𝙤𝙜𝙧𝙖𝙢—𝘢𝘤𝘵𝘪𝘷𝘦 𝘱𝘢𝘳𝘵𝘪𝘤𝘪𝘱𝘢𝘵𝘪𝘰𝘯 𝘪𝘴 𝘶𝘯𝘧𝘢𝘮𝘪𝘭𝘪𝘢𝘳.

🚨 None of these barriers are solved by tracking logins or minutes spent.

As Dr. Mario Weiss pointed out, this approach doesn’t solve the Real Problem —it just adds noise.

I’m also with Fabrice Pöhlmann—we should involve users from the start instead of measuring engagement after we’ve built the solution. If the product isn’t meaningful, no metric will fix that.

And Martin Möckel is spot on about explanatory videos—educating both patients & doctors on how and why to use DiGAs could remove one of the biggest adoption barriers.

So what should we be tracking instead to learn RealWorld impact?

  • 🎯 Engagement with core therapeutic components (𝗻𝗼𝘁 𝗷𝘂𝘀𝘁 𝗼𝗽𝗲𝗻𝗶𝗻𝗴 𝘁𝗵𝗲 𝗮𝗽𝗽)

  • 🌐 Patient behavior change beyond the app (𝗗𝗶𝗱 𝘁𝗵𝗲𝘆 𝘁𝗮𝗸𝗲 𝗮𝗰𝘁𝗶𝗼𝗻 𝗶𝗻 𝗿𝗲𝗮𝗹 𝗹𝗶𝗳𝗲?)

  • 🌟 Clinical outcomes linked to usage (𝗻𝗼𝘁 𝗷𝘂𝘀𝘁 “𝗳𝗲𝗲𝗹𝗶𝗻𝗴 𝗯𝗲𝘁𝘁𝗲𝗿” 𝘀𝘂𝗿𝘃𝗲𝘆)

If we actually want DigitalTherapeutics to deliver real value, measurement needs to go beyond “Did they log in?” So, what’s a better way to measure real world impact in DTx?
